When to Go Straight to Hospital
Most toothache needs a dental appointment, not a hospital visit. But if you have swelling that is spreading across your face or neck, swelling around the eye, difficulty breathing or swallowing, or a serious injury to your face or mouth, go to the nearest hospital emergency department or call 112. Do not wait for a dental appointment.
What Can Cause Tooth or Dental Pain?
Tooth pain is a symptom, not a diagnosis. The same feeling — a dull ache, a sharp twinge or throbbing at night — can come from different problems, and only an examination can tell them apart.
Cracked or damaged tooth
A cracked tooth may hurt when you bite, when you release the bite, or with temperature changes, and the pain may come and go. Cracks are not always visible.
Wisdom tooth problems
A wisdom tooth that has only partly come through the gum can become sore or swollen and hard to keep clean. The discomfort is usually felt towards the back of the jaw.
Pain after treatment or injury
A tooth can be sore after a filling, crown or other treatment, or after a knock. Some soreness can follow treatment, but pain that is severe, getting worse or not settling needs to be reviewed.
When Should You See a Dentist for Tooth Pain?
Book a dental appointment promptly if you have:
- Tooth pain that lasts, keeps coming back or is getting worse
- Pain that affects your eating, chewing or sleep
- Swelling in the gum, cheek or jaw
- A bump like a pimple on the gum, or a bad taste that will not go away
- A broken, chipped or cracked tooth
- Sensitivity to hot or cold that lingers
- Pain that began after dental treatment and is not settling
Brief, mild sensitivity is less urgent, but it is still worth mentioning at your next visit.
When Is Tooth Pain an Emergency?
Most toothache is not a medical emergency, but it should be checked. Some symptoms need urgent medical care rather than waiting for a dental appointment:
- Difficulty breathing
- Difficulty swallowing
- Swelling of the face, jaw or neck that is increasing quickly
- Swelling that reaches the eye or the area around it
- Fever together with significant swelling of the face or mouth
- A serious injury to the face, jaw or teeth
- Bleeding that does not stop
In these situations, go to the nearest hospital emergency department or call 112. A dentist can treat the cause once you are safe.
What Happens During a Pain Consultation?
The aim is to find where the pain is coming from before deciding on treatment. In general, a consultation for tooth pain includes:
- Your history — where the pain is, what it feels like, when it started, what sets it off and what eases it. Please also mention any medical conditions and the medicines you take.
- An examination — the dentist looks at your teeth, gums, existing fillings or crowns and bite, and may gently tap or press on a tooth, or check how it responds to temperature.
- Imaging where needed — a dental X-ray or other imaging can show what cannot be seen from the outside.
- An explanation — what was found, what your options are and what each involves, so you can decide.
Not every cause can be pinned down at the first visit. Sometimes a review or a further test is needed. If you have earlier dental X-rays or reports, bring them.

A toothache does not automatically mean a root canal, and one treatment does not suit every tooth. Depending on the examination, the next step may be:
Temporary Relief vs Treating the Cause
Tooth pain can ease for a while, or come and go, while the cause is still there. A quieter tooth does not always mean the problem has gone. While you wait for your appointment:
- Brush gently and keep the area clean.
- Avoid chewing on the painful side, and avoid foods and drinks that set the pain off.
- Make a note of when the pain happens and what triggers it. This helps your dentist.
- If you take regular medicines or have a medical condition, check with a doctor or pharmacist before taking anything for pain.
- Avoid placing tablets, oils or other home remedies directly on the tooth or gum.
These steps are only for comfort. They do not treat the cause, and they are not a reason to delay an appointment.
